Posting the sources I've found so far for gear that drops above the 265 legendary engram/faction reward threshold and hoping people have more I haven't found.

For those not yet familiar, legendary engrams and farming for faction reputation/packages can only get you so far as they only drop up to 265. Afterwards there are higher drops from certain activities and quests. As a tip if you're trying to achieve a high level you probably want to wait to do certain activites until you have already gotten most of your gear to 265 from legendary engrams, this way when you do the quests and guaranteed drops they will be higher than if you did these activities early. There's not a huge amount of sources at this point until the raid, trials and IB come out.

Sources of gear above 265:

Nightfall
Flashpoint
- check your milestones in the director
Story Quests and exotic quests such as the MIDA quest and Rat King
Competitive Crucible - I've had drops well over 265 in competitive, but only up to 265 in casual crucible. I'm not sure if there's a cap on this or if you just need to win to get the drops over 265. I know on my first win I received 2 legendary gear pieces above 265. On my next game, a loss, I got a 265 legendary. Destiny 2 is definitely more PvP oriented than the first game and I highly suggest going into competitive with a team.
Crucible Milestones - The "Call to Arms" milestone is a "Powerful Gear" drop above 265, but the other crucible milestone I only received reputation coins.
Exotics - this one is obvious, but it appears exotics can not be used to infuse legendary gear (anyone confirm?)

Other possible sources I need confirmed:
Vanguard strikes - Do these actually drop gear higher than 265 at the end?
Other Milestones - I completed all of mine, but may have forgotten some or not even unlocked some yet.

Not yet released:
Raid
Trials
Iron Banner

I threw this up pretty quickly, but just curious to see any other drops you guys are getting and if running strikes is worth it.

I missed the cayde treasure chests on my list, but all I remember getting was blues from the chest. Someone else probably has a better memory than I do.

Where it says you can't cross infuse classes. Does that mean if I have a 260 chest piece on warlock I can't infuse my Titan's chest piece with it? Or simply talking about the weapons, where you can't infuse a pulse rifle to a scout rifle despite being in the same weapon type?

I just want to note that flashpoint is a weekly.
