So This error has been occurring ever since I got my PS4 a little over year ago. I only play GTA 5 and the error occurs mostly with this game. It's completely random when it happens. I have no idea how to fix it so it would be awesome if anyone could help :smile:

I am going through the same, have tried everything from a Full Initialization to an HDD swap (from another PS4, so it is an original PS4 500GB Hard Drive) and still nothing.

Looks like the console is defective in some way...
