Is there a list or thread sticky with best ways to stay under the radar of rockstar?
Or a thread saying what WILL get you caught?

No matter what your modified console will be banned eventually. The best way to make your KV last is to pay for a reputable Stealth Server such as NiNJA.

Why? How do they search for modded xboxs?

There is no "search" done. There is an automated system that flags consoles running unsigned code. Stealth Servers keep you going longer, but you will eventually get banned no matter what in the end. Even if you only did invite only lobbies with friends, your KV will eventually get banned.
