With that manufacture date it should be a Trinity motherboard.

According to WeekendModder:

Slim S:

The nice thing about the Slim series is that there are only 2 possible motherboards within them:

  • Trinity (Most consoles with a MFR date of 07-2011 and BEFORE)
  • Corona (Most consoles with a MFR date of 8-2011 and AFTER)

That is a good start but the only way to confirm is to open the system and look at the Nand points. Trinity is 2 sets in parallel while the corona is 2 sets with one of them turned 90 degrees.
