It's not a guaranteed fix, but it might work. Go into your settings and make sure subtitles are enabled for multiplayer. Yes, you are technically playing the campaign...but co-op is a "multiplayer" experience.

It's been awhile, but if memory serves me right there will only be a time stamp if they were unlocked online. If they were unlocked offline, there won't be a time stamp on the achievement. There's nothing to worry about, you're gold.

If you're still looking for a solution to this, to play burned games on a RGH console without flashing the drive, you will need to look for a plugin "netiso" and have it set as one of your active plugins.
If there is nothing else on the hard drive, it's most likely that you accidentally deleted the games in some way. The only thing you can do at this point is to redownload everything that you can remember was on it originally.

I had a similar issue going on and what solved it for me was after enabling/disabling the options I wanted, I pressed "RB", went down to my HDD, and pressed "X" to save the ini. Did that, rebooted console, checked everything and it was all still set to what I wanted.
